1. Recommend mode of transport from Chandigarh?

Bus is good if you are ok.
Many types of buses available.
Volvo / Luxury / AC / Non AC.
Roads are good. It is less than 250km.

2. Recommend hotel accommodation for two price between Rs. 3000-Rs. 4000 near Harmandir Sahib?

Many available from 1500 plus. Check oyo or other hotels on Google Maps also.
Main parking area to Main premises is only for pedestrians.
Better plan for some nearby area of Parking side.
Enjoy return and other rides from there. Side of Golden Temples may have traffic jams if going these days.

During 12 November 2019, there can be heavy rush time.
In Christmas & New Year Holidays also same Heavy Rush will be there.

3. A good cab rent for City tour?

Can't say where do you want to roam in city.
For Wagha Border any Taxi wala will be ready to go for evening.
Else all other points can be covered on Auto / E Rickshaw / Rickshaw also. Shared taxi also available for famous areas.
Auto is good for small city roaming at main food joints.
But it is better to hire an auto or taxi from station itself.
Can't comment on rates.
