how to fill my wife's passport form

i want to do application for my wife's passport.
can anyone guide me how to fill below category in form.

main catagory:- Place of Birth
sub catagory:- village or town or city:- it should me birth of place or where we live.

main catagory:- Family Detail
sub catagory:- father and mother given name. should i write her mother and father name.

Manin catagory:- emergency contact detail
name and address:- whose name and address?

please let us know .

When it is her Passport - No details of yours except where asked for 'Spouse' Name / Address etc.

sub catagory:- village or town or city:- WHERE SHE WAS BORN (No documentary evidence shall be required)

main catagory:- Family Detail
sub catagory:- father and mother given name. HER FATHER NAME and HER MOTHER NAME

Manin catagory:- emergency contact detail
name and address:- GIVE YOUR NAME, ADDRESS and PHONE No.
