While the form is correct, this year we had filled out another application alongside it as well. Not sure whether or not filling it is compulsory or not. But as far as the form is concerned, it should be available with most of the guest houses in Leh (Shanti Guest House had it) and at the very bottom where it says, “Copy for the Information”, that is where one needs to fill out guest house name.
This form is also available at the photo copy shop behind the DC Office i.e. you come up at the fork where the left route takes you to DC Office Parking, while the right one goes ahead, well just take that and take the first right afterwards. There are two photocopy shops there which have both of the forms, and keep in mind you also need to attach a photocopy of your id proof with the application as well.

Yogesh, which is the other form one has to fill?? And what is the information it requires?

It was more like a replacement for the written application one used to give earlier and is supposed to be kept by the DC Office for record, while this application is given back as permit.
Information required is the same as this form, dates, details of those traveling and of course, places to be visited.

Dear Yogesh,
What would be the identity proof document for this form? In my case, I cannot provide photocopies of my service identity card but do possess standard documents like a passport, a driving license card and a PAN card.
May I request you to recommend a document which all members of my team would possess most commonly so that I can prep them to carry it on our trip this June?

Driving license would be alright, but it is generally better to provide something like Voter ID Card or PAN card, because at times government officials can be a little finicky about things.
