Tips/Guidelines for "Change of Name" in 'Official Gazette of India'
1)
Advertise the change of name in local leading newspaper (better get it done in 2 newspapers)
For this, be ready with an affidavit (on Rs.10 stamp paper and attested by Notary) stating the reason of name change while mentioning old name and residential address and father/husband name, etc.; and then mentioning the new name and residential address and father/husband name, etc.!!
Get in touch with newspaper advertising agencies and get it published. Buy 10-15 copies of each newspapers for future references!!
Tear up whole page (date & name of paper should be clearly visible) from newspaper and highlight your publication/announcement.

Make another affidavit (on Rs.10 stamp paper and attested by Notary) with full details of father/husband name, residential address, etc.

Two passport sized photopgraphs. Paste them on an A4 paper, get them attested by Notary.

One declaration to be made in duplicate (2 copies, to be precise!) as per the specimen provided by the Deptt. of Publication, New Delhi.

Both copies of declarations are to be signed by 2 witnesses as well...!!

Printing charges for publication of name change is Rs. 700 for person residing in India. For those who are residing abroad, it is Rs. 2300 (including Air-Mail charges!!) Make a Demand Draft for the same.

With this, you'll get ONE copy of gazette. And one can also get (max) 2 extra copies of Gazette by adding up Rs. 35 for each in the Demand Draft.
Send a single draft of Rs. 770 or Rs. 2370, as the case may be.

NONE OF YOUR DOCUMENT WILL BE RETURNED... BE IT ORIGINAL / PHOTOCOPIED / ELSE...!!!!!

You can go in person, or send it thru Registered Post. Visiting hours are 10AM-1PM & 2PM-3PM on any working day. Saturdays are offdays!

You'll not get any acknowledgement/confirmation of receipt, etc. immediately. One is expected to call up to them after 12-15 days to ask for status and acknowledgement!
You'll have to give the reference of your old & new names only... coz you won't be having any other thing to ease up the matter!

If all goes well, they will send you 3 copies of Gazette thru post!

Now, wherever you need to attach a "Change of Name" declaration... just attach the Notary Attested copy of Gazette... and you're done!
